Please fill in the columns carefully:

• kol. 8 Family name ,given names -  NO REVERSE !!!
• kol. 11 Date and place of birth - date in uniform format [year-month-day]
• kol. 6 Nature and no of identity document - please enter the type of ID document before its number, as in the example list [Pass. / ID]
